About the Role We are looking for an experienced 245D Designated Coordinator to oversee and support our team of Direct Support Professionals (DSPs) in delivering high-quality services that meet the unique needs of our clients. As a 245D Designated Coordinator, you will play a pivotal role in ensuring compliance with 245D standards, developing and implementing individualized program plans, and maintaining accurate documentation. Key Responsibilities Develop and Update Plans: Create and revise CSSP Addendums (CSSPA) and Individual Abuse Prevention Plans (IAPP) to reflect clients' needs and goals, ensuring that all plans are person-centered and tailored to meet individual requirements. Regulatory Compliance: Ensure that all services and documentation comply with MN Statute 245D licensing requirements and legal standards, maintaining a thorough understanding of regulatory requirements and best practices.
