Key Responsibilities
Conduct thorough assessments to evaluate patient needs.
Develop tailored care plans that align with individual health goals.
Coordinate effectively with physicians, hospitals, and various healthcare providers.
Educate and guide patients and their families throughout their healthcare journey.
Advocate for patients to ensure they receive optimal care.
Maintain precise and updated patient records.
Required Skills
Current Registered Nurse (RN) license.
Minimum of 2 years experience in case management, including managed care or home care.
Strong communication and interpersonal abilities.
Capacity to work independently as well as collaboratively within a team.
Excellent organizational and time management skills.
Bilingual skills (Spanish, Russian, or Chinese) are advantageous.
Qualifications
Valid and active Registered Nurse (RN) license.
At least 2 years of relevant experience in case management or a related field.
