CCM is a Medicare-sponsored program designed to improve health outcomes for patients with chronic conditions by coordinating care, offering ongoing support, and enhancing communication between providers and patients.
• *Position Overview:**

We are seeking an experienced Pharmacy Technician to join our remote team as a full time clinical staff member. In this role, you will be responsible for completing patient onboardings before passing them to a licensed pharmacist for ongoing care. You will interact directly with patients, explaining processes, collecting necessary information, and ensuring they are well-prepared for their chronic care management journey.
• *Key Responsibilities:**
• Perform patient onboarding remotely, including gathering patient information, reviewing medications, and explaining the chronic care management process.
• Communicate clearly and effectively with patients to ensure they understand their role in the care plan.
• Collaborate with pharmacists to ensure smooth transitions for patient care.
• Maintain organized and accurate patient records using Google Drive.
• Utilize clinical knowledge to assess patient needs and provide appropriate guidance.
• Handle sensitive patient information in compliance with HIPAA and company policies.
• *Qualifications:**
• Texas Pharmacy Technician Registration & Certification current and in good standing.
• Minimum of 5 years of experience as a Pharmacy Technician.
• Strong clinical understanding of chronic disease states and medication management.
• Experience in patient-facing roles, with the ability to communicate medical information clearly.
• Bilingual skills (English and Spanish strongly preferred).
• Proficiency in using Google Drive for organizing and sharing patient data.
• High attention to detail and commitment to patient confidentiality.
• Quiet, distraction-free, HIPAA-compliant workspace with reliable internet with minimum speeds of 100 mbps download and 10 mbps upload.
• 32 to 40 hours per week during traditional business hours to work independently, prioritize tasks, meet deadlines, and manage time effectively in a remote setting.

Job Type: Full-time

Pay: $18.00 - $20.00 per hour
