CCM is a Medicare-sponsored program designed to improve health outcomes for patients with chronic conditions by coordinating care, offering ongoing support, and enhancing communication between providers and patients.
 *Position Overview:**
We are seeking an experienced Pharmacy Technician to join our remote team as a full time clinical staff member. In this role, you will be responsible for completing patient onboardings before passing them to a licensed pharmacist for ongoing care. You will interact directly with patients, explaining processes, collecting necessary information, and ensuring they are well-prepared for their chronic care management journey.
 *Key Responsibilities:**
 Perform patient onboarding remotely, including gathering patient information, reviewing medications, and explaining the chronic care management process.
 Communicate clearly and effectively with patients to ensure they understand their role in the care plan.
 Collaborate with pharmacists to ensure smooth transitions for patient care.
 Maintain organized and accurate patient records using Google Drive.
 Utilize clinical knowledge to assess patient needs and provide appropriate guidance.
 Handle sensitive patient information in compliance with HIPAA and company policies.
 *Qualifications:**
 Texas Pharmacy Technician Registration & Certification current and in good standing.
 Minimum of 5 years of experience as a Pharmacy Technician.
 Strong clinical understanding of chronic disease states and medication management.
 Experience in patient-facing roles, with the ability to communicate medical information clearly.
 Bilingual skills (English and Spanish strongly preferred).
 Proficiency in using Google Drive for organizing and sharing patient data.
 High attention to detail and commitment to patient confidentiality.
 Quiet, distraction-free, HIPAA-compliant workspace with reliable internet with minimum speeds of 100 mbps download and 10 mbps upload.
 32 to 40 hours per week during traditional business hours to work independently, prioritize tasks, meet deadlines, and manage time effectively in a remote setting.
