We're seeking a highly skilled and experienced Remote RN Admissions Case Manager to join our team on a part-time/as-needed basis (PRN). As a key member of our care coordination team, you'll play a vital role in ensuring seamless transitions and optimal care for our patients. As a Remote RN Admissions Case Manager, you'll be an integral part of our team, working closely with healthcare professionals, patients, and families to ensure that our patients receive the right care, at the right time, and in the right setting. Key Responsibilities Direct and coordinate admissions and transfers to, from, and within the hospital, ensuring timely and efficient patient flow. Facilitate professional relationships between unit leadership, operational coordinators, nursing staff, and interdisciplinary teams to provide patient/room assignments based on patient diagnosis, acuity, MD orders, and patient flow. Manage aspects of the planning process in anticipation of the patient's movement through the healthcare system, ensuring continuity of care. Interact with managed care/third-party payers to obtain financial certification for appropriate reimbursement and minimize reimbursement denials. Serve as a contact person for the rehabilitation team, internal admissions team, clients, and external sources to enhance patient outcomes. Educate clients, family members, or significant others, referral sources, hospital personnel, external payers, and internal/external case managers about rehabilitation program benefits and services. Adapt behavior to the specific patient population, demonstrating respect for privacy, cultural sensitivity, and effective communication. Perform other related duties as required, staying up-to-date with current federal, state, and local laws, accreditation standards, and regulatory agency requirements. Essential Qualifications To excel as a Remote RN Admissions Case Manager, you'll need: A Registered Nurse diploma and current Registered Nurse license in the state of practice. At least 1 year of hospital-based experience in the delivery of patient care. Proficiency in using computers, software, and web-based applications. Effective verbal and written communication skills, with the ability to present information clearly and professionally. A good understanding of business/finance principles. The ability to work a flexible schedule, including 24/7, weekend, holiday, and on-call availability (as needed). Preferred Qualifications While not required, the following qualifications will give you a competitive edge: An Associate's degree in nursing. Experience in case management or utilization review. Certifications such as Commission for Case Manager (CCM), Certified Professional Utilization Review (CPUR), Certified Professional in Utilization Management (CPUM), or Certified Professional in Healthcare Management (CPHM).
