Requirements
Active RN license in the state of residence
Minimum of 3 years of clinical experience, preferably in a home health, hospice, or case management setting

Reliable high-speed internet connection
Comfortable using telehealth platforms and technology
Ability to work independently and as part of a team
Responsibilities
Provide telephonic and virtual care to patients, offering support and guidance
Educate patients on their health conditions and treatment plans
Coordinate care with other healthcare providers to ensure continuity and quality of care
Monitor patient progress and adjust care plans as necessary
Document patient interactions and maintain accurate records in the EHR system
Participate in ongoing training and professional development activities
