As an Access Nurse, your work will focus on coordinating patient flow. In this position, you will facilitate hospital transfers and direct admissions, determine daily bed resources and oversee bed management logistics for the health system. This position is vital in gathering accurate data to enhance efficiency and promote patient safety and need prioritization.

This is a PRN, remote position. Caregivers in this role work a variety of shift lengths (4‑, 8‑, 10‑, or 12‑hour shifts) based on availability, following an initial in‑person training period. Team members are required to work a minimum of 48 hours every six weeks, including at least one weekend shift.

A caregiver who excels in this role will:
• Serve as the primary point of contact for telephone patients with a variety of health or illness questions.
• Utilize effective and professional communication, clinical knowledge and nursing assessment skills to quickly triage patient status.
• Provide intake, assessment and triage for internal and external hospital transfers and direct admissions on request in accordance with standard operating procedures.
• Elicit information from referring providers, review patient clinical information and assess patient status for appropriate placement.
• Collaborate with health care providers and hospital leadership to optimize patient outcomes utilizing clinical expertise and incorporating nursing and management experience with critical thinking skills and business intelligence tools to achieve optimal outcomes both clinically and financially for the patients and the organization.
• Apply solid understanding of the complexities of the patient status and the process and capabilities of each system hospital to seamlessly facilitate and achieve enterprise bed placement.
• Provide oversight of all bed management functions.
• Function at any point in the patient flow process designated to ensure the safe and efficient placement of patients and effective utilization of resources.
• Demonstrate cost effective management of enterprise bed resources to enable the organization to provide quality care to patients.

Minimum qualifications for the ideal future caregiver include:
• Graduate of an accredited school of Professional Nursing
• Current state license as a Registered Nurse (RN)
• Three years of recent progressive clinical experience in an acute care setting

Preferred qualifications for the ideal future caregiver include:
• Bachelor's of Science in Nursing (BSN)
• Two years of supervisory experience in a healthcare setting
• Computer proficiency in Microsoft applications, bed management databases and Epic
• ICU, ED, Critical Care and/or Bed Management experience

Physical Requirements:
• Requires sitting for long periods of time, multitasking, manual dexterity to operate computers and normal or corrected vision.
• Requires working under stressful and emergency situations with the ability to maintain composure in any situation in order to provide superior customer service and ensure proper patient safety and placement.
• Must be able to perform multiple functions simultaneously without supervision and learn new practices promptly.

Personal Protective Equipment:
• Follows standard precautions using personal protective equipment as required.

Pay Range

Minimum hourly: $41.00

Maximum hourly: $41.00
