Nurse Practitioner - CLINICAL NURSE SPEC-GERONTOLOGY (Full Time; Honolulu)

Responsibilities
ā¢ Coordinates case management for members in long-term care facilities.

ā¢ Provides consultation and direct care to geriatric patients in inpatient, outpatient, and home care settings.

ā¢ Conducts intake assessments and develops individualized plans of care for members in long-term care facilities.
,
ā¢ Performs annual physical exams, timely certifications, and routine follow-up visits in collaboration with physicians.
,
ā¢ Documents all care provided, including observations, treatments, and communications according to standards.
,
ā¢ Orders and monitors laboratory and diagnostic tests.
,
ā¢ Educates patients and families about health status and treatment options.
,
ā¢ Collaborates with hospitals and clinics in program development and quality improvement initiatives.
,
ā¢ Identifies needs for inservice programs and participates in educational planning and evaluation.
,
ā¢ Acts as a liaison for facility and family concerns related to long-term care.
,
ā¢ Participates in patient care conferences with healthcare team members.
,
ā¢ Represents Kaiser Permanente at community events.
,
ā¢ Takes call for selected long-term care facilities on a rotation basis.
Requirements
ā¢ Minimum three years of direct patient care nursing experience, with one year in long-term care or geriatric specialty.

ā¢ Master's degree in nursing with a specialty in gerontological, adult, family, or acute care.
,
ā¢ Nurse Practitioner-Family Certification or equivalent certification required.
,
ā¢ Advanced Practice Registered Nurse License (Hawaii) and Registered Nurse License (Hawaii) required at hire.
,
ā¢ Basic Life Support certification from American Heart Association required at hire.

ā¢ Valid Hawaii APRN Recognition with Prescriptive Authority.
