Key Responsibilities:
Serve as primary care manager for assigned members, conducting holistic assessments, and developing personalized care plans to address complex health and social needs.
Collaborate with our multidisciplinary team, including physicians, nurses, and other healthcare professionals, to ensure seamless care coordination and transitions.
Provide education, support, and guidance to patients and their families, empowering them to take an active role in their healthcare journey.
Utilize advanced clinical judgment and critical thinking skills to identify high-risk patients, develop targeted interventions, and prevent hospitalizations.
Travel 50-75% within the Fredericksburg, Virginia area, conducting face-to-face assessments, and providing care management services to our members.
Requirements:
Minimum of 3 years of professional/clinical nursing experience, with a preference for experience in care coordination.
2+ years of telephonic and/or field care management experience.
Active Registered Nurse (RN) license in the state of Virginia.
Strong organizational, communication, and problem-solving skills, with proficiency in electronic health records (EHR) systems.
Ability to travel 50-75% within the Fredericksburg, Virginia area.
