Job Summary
As a Hybrid Behavioral Health Nurse LVN/LPT, you will play a vital role in our crisis intervention and stabilization services at the Valley Star Crisis Walk-In Center. You will provide direct client care, administer medications, and supervise nursing activities performed by professional and para-professional nursing personnel. Your expertise and compassion will help individuals in crisis receive the support they need to navigate challenging situations and transition to the next level of care.
Key Responsibilities
Provide direct client care, including administering medications, monitoring vital signs, and documenting client information
Supervise nursing activities performed by professional and para-professional nursing personnel
Order, prepare, administer, and document the administration of medications according to physicians' orders
Confirm delivery of all ordered medications from the pharmacy on shift worked
Communicate pertinent information verbally and in writing to all team members
Write weekly summaries on assigned clients
Maintain accurate and up-to-date nursing documentation in client medical records
Participate in crisis intervention and stabilization services, including rapid mental health assessments and crisis intervention
Collaborate with multidisciplinary teams to develop and implement individualized treatment plans
Requirements and Qualifications
Essential Qualifications:
Graduation from an accredited school of licensed vocational nursing or psychiatric technology
Valid California LVN or LPT license
Current CPR certification
First Aid certification from a qualified agency (e.g., American Red Cross)
Current California Driver's License
Applicants must have attained the age of 18 years
Preferred Qualifications:
Associate degree in nursing or a related field
One (1) year of service experience treating the mentally ill in an inpatient setting
One (1) year of medication administration experience
