Role Description
SSM is looking for an experienced nurse with Hospice/Liaison or Case Management experience. The selected individual will be responsible for:
ā¢ Meeting with patients/families in person to discuss continuum of care services.
ā¢ Meeting/communicating with referral sources (case management, social work, clinics, providers, SNF/ALF facilities, etc).
ā¢ Care coordination between continuum of care departments, such as branch departments, business development, etc.
ā¢ Eligibility/Chart Review to determine appropriateness for continuum of care services.
This position is primarily a weekend position and Fridays. The hours are 8am-4:30pm with no on-call requirements. This is a remote position, covering our entire service areas of WI, IL, OK, and MO. Multistate licensure required; it is not required at time of hire but would need to work towards it started (this is reimbursed by SSM).
Qualifications
ā¢ Graduate of accredited school of nursing or education equivalency for licensing
ā¢ No experience required
Requirements
ā¢ Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
ā¢ Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements.
ā¢ Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
ā¢ Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc.
ā¢ Frequent keyboard use/data entry.
ā¢ Occasional bending, stooping, kneeling, squatting, twisting and gripping.
ā¢ Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
ā¢ Rare climbing.
