As a Health Coach on our Healthy Outcomes Team, you will play a crucial role in helping individuals achieve their health goals and live healthier lives. We are looking for a passionate and driven individual who is dedicated to making a positive impact on others. If you have a strong background in health and wellness and a desire to empower others to make positive changes, we want you to join our team. Provide personalized coaching and support to individuals in achieving their health and wellness goals Develop and implement tailored action plans for each individual based on their specific needs and goals Conduct regular check-ins and follow-ups with individuals to track progress and provide ongoing support Educate individuals on healthy lifestyle choices and provide resources for maintaining a healthy lifestyle Collaborate with healthcare providers and other members of the Healthy Outcomes Team to ensure comprehensive and coordinated care for individuals Stay up-to-date on current health and wellness research and trends to provide the most relevant and effective coaching strategies Utilize motivational interviewing techniques to encourage and empower individuals to make positive changes Maintain accurate and organized records of all coaching interactions and progress towards health goals Actively participate in team meetings and trainings to continuously improve coaching skills and strategies Serve as a positive and professional representative of CVS Health, promoting our commitment to improving the health and well-being of our customers.
