Facilitated Telehealth Coordinator- Broward, FL
About the position Responsibilities • Travel to patient homes or primary care offices to assist patient and/or caregiver with connecting to the tele-health visit - typically up to four visits/day • Facilitate communication between providers and their patients including assisting patients use of technology to conduct virtual visits with other providers on the team • Assess environment regarding safety concerns or potential hazards • Communicate with the patient care team regarding any patient questions and needs such as medications, Community Based Organizations. • Coordinate telehealth services and ensure seamless delivery of care to patients in various settings: home, clinic, living communities (ALFs, etc.) • Provide education and support to patients and caregivers on how to use telehealth technology effectively • Maintain accurate and up-to-date records of telehealth appointments and patient data • Collaborate with IT and technical support teams to resolve any telehealth system issues • Conduct training sessions to educate healthcare staff on telehealth best practices and workflows • Collaborate with cross-functional teams to identify technology needs and opportunities for improvement • Troubleshoot and resolve technology issues in a timely manner to minimize downtime.
