Job Summary: Telephonic RN Nurse Care Manager

We are seeking an experienced and compassionate Telephonic RN Nurse Care Manager to join our team in Houston, TX. As a Nurse Care Manager, you will play a vital role in enhancing the health outcomes of our patients by managing their care and addressing barriers to access. You will work collaboratively with our care team to develop and implement personalized care plans, coordinate care services, and provide education to patients and their caregivers.

Key Responsibilities:


Coordinate care services with pod leaders to ensure comprehensive and tailored care for patients throughout their healthcare journey
Develop and manage personalized care plans in collaboration with the care team to address care gaps and ensure access to care
Coordinate transitions of care for patients across the continuum to prevent avoidable hospital admissions and ensure seamless care delivery
Facilitate High Risk Huddles and ensure follow-up actions are completed to prioritize patient needs and improve outcomes
Prioritize patients based on the severity and urgency of their conditions to ensure timely and effective care
Review medical records to identify gaps in care and coordinate services with the care team to manage these issues
Regularly update patient care plans to reflect changing needs and priorities
Perform thorough nursing assessments via telephone to maximize or improve current health outcomes
Provide education to patients and/or their caregivers on disease management, medication, health maintenance, and disease prevention to promote self-management and improve health outcomes
Demonstrate strong clinical skills, critical thinking abilities, and effective communication in interactions with patients, caregivers, providers, and fellow care team members
Document necessary interactions, assessments, updates, etc. in patient medical records according to processes and guidelines
Serve as a liaison between patients, providers, resources, etc. to ensure seamless care delivery and facilitate communication during transitional experiences


Requirements:

Essential Qualifications:


Active Registered Nurse license
2+ years of care management experience in community, health plan, or hospital systems
Strong clinical skills and proactive thinking
Effective communication skills
Ability to perform extensive telephone assessments
Knowledge of Medicare regulations and home care and hospice standards
Experience with small group presentations and teaching/training
Excellent interpersonal, written, and oral skills
Working knowledge of computer programs (email, Word, Excel, PowerPoint, etc.)
Ability to manage time effectively to ensure all duties and documentation requirements are completed in a timely manner


Preferred Qualifications:


Bachelor of Science in nursing or related field
May be required to obtain multi-state licensing
Strong knowledge of population health, quality measures, care gap closure, and value-based care models
