About the Role We are seeking an experienced and dedicated Triage LVN to join our team for weekend on-call duties. As a Remote Triage Weekend Evening/Overnight LVN, you will play a pivotal role in our patient care continuum, providing critical support and guidance to patients, caregivers, and facilities during evening and overnight hours. Key Responsibilities Answer all incoming phone calls during scheduled on-call hours and respond to patient, caregiver, and facility inquiries with empathy and professionalism. Coordinate with nursing staff to dispatch field nurses (runners) for patient visits as necessary, ensuring timely and appropriate care. Provide emotional support and practical care guidance to families and caregivers, addressing their concerns and needs with compassion and understanding. Utilize triage protocols to assess the level of urgency and determine the best course of action for patient care, making informed decisions that prioritize patient well-being. Document all triage activities, calls, and interventions using company-provided software and communication tools, maintaining accurate and detailed records. Communicate with the interdisciplinary team regarding patient needs and updates, fostering a collaborative approach to patient care. Essential Qualifications An active LVN license in the state of California is a critical requirement for this role. A minimum of 1 year of experience in hospice care or a similar healthcare setting is necessary, with a strong background in patient assessment, care planning, and triage. Strong communication and organizational skills are essential, with the ability to work independently and make informed decisions during triage situations. Proficiency in using electronic medical records and communication software is preferred, with a willingness to learn and adapt to new systems. Preferred Qualifications Prior experience in a remote or on-call setting is highly desirable, with the ability to work effectively in a standalone environment. Knowledge of hospice care principles, practices, and regulations is advantageous, with a commitment to staying updated on industry developments. Bilingual or multilingual skills are a plus, enabling more effective communication with diverse patient populations.
