Job Overview We are seeking an experienced and skilled Hospice Case Manager - RN - PRN to join our team in Beaumont, TX. As a member of our clinical team, you will lead by example, delivering professional nursing care to our patients in accordance with established plans of care. You will be responsible for completing comprehensive assessments, formulating individualized plans of care, and providing skilled interventions to achieve realistic outcomes. Key Responsibilities Deliver professional nursing care to patients in accordance with established plans of care Complete comprehensive subjective and objective assessments of patient status, including physical, psychosocial, and environmental parameters Formulate individualized plans of care that incorporate analysis of assessment data and modify/update as needed Provide skilled interventions aimed at achieving realistic outcomes within a specified time period Ensure thorough and accurate documentation of all clinical data, including admission through discharge, physician's orders, skilled nursing visits, and changes in conditions Perform clinical chart record reviews as requested and perform on-call responsibilities as assigned Determine if patients meet admission criteria and assist in the assignment of qualified personnel Assist physicians in developing plans of care and make necessary revisions based on patient conditions and needs Prepare clinical progress notes and review documentation on a continual basis to ensure compliance with hospice documentation standards Adhere to the organization's patient care policies, including the implementation of patient care plans Essential Qualifications To be successful in this role, you will need: Graduation from an accredited school of nursing, with a Bachelor's degree preferred One (1) year of experience as a nurse in hospice care or post-acute setting Current Registered Nurse licensure (in good standing) in the state(s) of practice Current CPR certification Valid driver's license and automobile insurance Strong clinical skills, with the ability to work in a collaborative and solo contributor capacity Experience with EHR systems and their implementation/optimization in complex clinical settings Proficiency with federal, state, and governing agency regulations Preferred Qualifications While not essential, the following qualifications are preferred: Management experience, although not required Experience supervising hospice aides Strong ability to delegate selected nursing tasks Outstanding presentation, writing, and communication skills Creative thinker who embraces thought leadership Ability to make challenging decisions and take responsibility for them Career Growth Opportunities and Learning Benefits At Intrepid USA, we are committed to the growth and development of our team members.
