Diabetes Care Coordinator

Remote Full-time
The ideal candidate will work with the Endocrinology and OB teams to support the care of pregnant patients with diabetes, and assist with tracking, monitoring, outreach, and supporting all patients involved in this program.

Essential Functions

Resource Connection: Serving as a point of contact for patients with diabetes, providing education, coordination, and support for ongoing participation in care.


Treatment Planning: Supporting individualized treatment plans in collaboration with patients and other healthcare team members. Assisting patient and clinical teams in obtaining prior authorizations for medication and CGM supplies.


Advocacy: Advocating for patients' needs and ensuring they receive the necessary support and services. Coordinating CGM initiation, training, and follow-up, ensuring timely device placement and data uploads.


Documentation: Maintaining accurate and timely documentation of patient interactions, assessments, and interventions. Collecting, organizing, and maintaining patient data related to glucose monitoring, lab results, and pregnancy outcomes in compliance with HIPPA regulations.


Collaboration: Working closely with other healthcare professionals, such as Physicians, Nurse Practitioners, Diabetes Educators, and Staff to ensure a coordinated and comprehensive approach to patient care. Supporting quality improvement initiatives and contributing to periodic progress reports as well as participating in team meetings.


Education: Associates Degree


Requirements:

Experience in prior authorization workflow, utilizing various means, including electronically, fax, paper, telephone, and maintaining online spreadsheet, is preferred


Experience in data collection and tracking, is preferred


Experience in healthcare settings, particularly in ambulatory or outpatient care, is preferred


Familiarity with electronic health records (EHR) and spreadsheet software, is preferred


Excellent communication, interpersonal, and organizational skills, is essential



Schedule: Monday-Friday, 9AM-5PM with options for some remote access

Working Conditions and Physical Effort:

Manual Skills : Significant (more than 50%) portions of daily assignments involve application of manual skills requiring motor coordination in combination with finger dexterity.
Physical Effort: Some (less than 50%) portions of daily assignments involve prolonged standing, transporting patients, material, and equipment, or lifting, moving, or carrying heavy (over 30 lbs.) materials for intermittent periods throughout the day.
Physical Environment: Some (less than 50%) portions of daily assignments involve exposure to dirt, odors, noise, or some work is performed with exposure to temperature/weather extremes. Working surfaces may be unlevel, slippery, or unstable.
Work Schedule: Work may require evening, night, holiday, or weekend assignments on occasion.
Occupational Risks: Some (less than 50%) portions of daily assignments involve occupational risk, such as cuts, burns, exposure to toxic chemicals, injuries from falls, or back injury sustained with assisting in moving/lifting/positioning patients, equipment, or materials.
Bloodborne Pathogens Exposure: OSHA Category III - Exposure not anticipated. Work does not involve exposure to blood, body fluids, or tissue, and does not require performing unplanned exposure tasks.
Safety Equipment: Job does not require the use of safety equipment, such as HEPA masks, for prevention of infection.
Ergonomics Risks: Significant portions (more than 50%) of daily assignments involve ergonomic risk.
