The BWH ACO Social Worker is an integral member of the multidisciplinary team for patients enrolled in the MGB Accountable Care Organization (MassHealth ACO), providing assessment, intervention, and advocacy for patients with complex medical and psychosocial needs. The MGB ACO is part of a statewide effort to restructure the care delivery system in ways that improve health outcomes in vulnerable populations. To this effort, the Social Worker will provide clinical care management aimed at engaging patients in primary and behavioral health care and community services in order to coordinate care and reduce acute care needs. . This position requires a high degree of flexibility, creativity, independence, and knowledge of the health care system and community resources to maximize the emotional, social, and physical well-being of patients.

PRINCIPAL DUTIES AND RESPONSIBILITIES:
• Use clinical skills and knowledge to build relationships with patients of diverse backgrounds and clinical presentations
• Conducts visits virtually (by phone or video conference) and in person (in primary care or in the community) as needed to help patient achieve goals
• Use clinical skills and intervention to continuously assess patients’ needs and to develop and advance care plan
• Apply a systems framework to understand the impact of micro, mezzo, and macro systems and systems of oppression on patients/families/communities. Use this knowledge to educate others involved in the patients’ care.
• Engage with patients and providers around discharge planning upon admission to inpatient facilities
• Address issues related to social determinants of health
• Identify community services and supports and facilitate strong connections between patients and community providers
• Leverage ACO programs to address patient needs; assist in oversight and collaboration with ACO programs.
• Advocate on behalf of patients and their families to gain access to services and resources.
• Meet with supervisor regarding key program dashboard measures such as program utilization and capacity as well as any barriers or concerns
• Document all relevant patient/collateral contact information in the patient’s electronic medical record; maintain communication with care team via in-basket messaging, email, phone, meetings etc.
• Collaborate with local and central leadership to improve integrated behavioral health management and achieve quality goals
• Participate in practice outreach and educational presentations, as needed
• Complete other projects and duties, as needed and specified by the supervisor

Qualifications

EDUCATION:
• Master’s of Social Work Degree from an accredited program required.

Licensure:
• Current Massachusetts Licensed Independent Clinical Social Worker (LICSW) required.
• Will consider LCSW or LMHC and Master's in mental health counseling.

EXPERIENCE:
• Minimum of 2 years of experience in a community or health setting.
• Candidates must demonstrate the ability to understand the role of social workers in a health care setting and to work in a complex, fast-paced environment.
• Must have demonstrated ability to work with medical professionals and individuals experiencing complex mental health challenges and their families.
• Must know community resources, crisis intervention skills, and the ability to work with a multidisciplinary team.

SKILLS & COMPETENCIES REQUIRED:
• Knowledge, skills, and ability to assess the patient’s need for services.
• Knowledge of community agencies/resources.
• Knowledge of specific medical/psychiatric/substance use illnesses, procedures, and treatments, related medications (i.e., psychotropic), acuity versus chronicity
• Comfort with a patient-centered approach that allows the patient’s desires around behavioral change to inform the plan of care.
• Ability to advocate/negotiate systems for/with patients.
• Ability to maintain effective working relationships with patients/families and staff.
• Excellent interpersonal and written and verbal communication skills
• Ability to work well in interdisciplinary teams including health care providers and office staff, representatives from Primary Care, Psychiatry, Social Work, and Population Health Management, and to build collaborative relationships
• Ability to work independently with minimal supervision
• Excellent advocacy and collaboration skills
• Excellent organizational skills
• Strong computer and database skills
• Maintains patient/family confidentiality

FISCAL: This is full time grant funded position with the possibility of extension pending funding renewal.

Where we work: This is a hybrid position with approximately 20 hours of remote work and 20 hours of in-person work.
