Bilingual Analyst, Case Management – Field – Manatee County, Ruskin, Apollo Beach or Sun City FL

They are seeking a self-motivated and detail-oriented Bilingual Analyst, Case Management Coordinator to manage a caseload of supportive and medically complex members, ensuring effective care coordination and optimal healthcare outcomes. Responsibilities Effectively manage a caseload that includes supportive and medically complex members Utilize critical thinking and judgment to collaborate and inform the case management process, in order to facilitate appropriate healthcare outcomes for members by providing care coordination, support and education for members through the use of care management tools and resources Conduct comprehensive evaluation of Members using care management tools and information/data review Coordinate and implement assigned care plan activities and monitor care plan progress Conduct multidisciplinary review in order to achieve optimal outcomes Identify and escalate quality of care issues through established channels Utilize negotiation skills to secure appropriate options and services necessary to meet the member’s benefits and/or healthcare needs Utilize influencing/ motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behavior changes to achieve optimum level of health Skills Bi-lingual in English & Spanish required 1 + year of experience in behavioral health or a long-term care setting Must reside in Manatee County Florida; Ruskin, Apollo Beach or Sun City Willing and able to travel 50-75% of their time to meet members face to face in Tampa or surrounding areas Bachelor's Degree required Ability to multitask, prioritize and effectively adapt to a fast-paced changing environment Effective communication skills, both verbal and written Managed Care experience Computer proficiency in Microsoft Word, Excel, and Outlook required Case management and discharge planning experience Degree in Social Work, Behavioral Health, or Human Services field of study preferred Master's degree Preferred Benefits Affordable medical plan options 401(k) plan (including matching company contributions) Employee stock purchase plan No-cost programs for all colleagues including wellness screenings Tobacco cessation and weight management programs Confidential counseling and financial coaching Paid time off Flexible work schedules Family leave Dependent care resources Colleague assistance programs Tuition assistance Retiree medical access Company Overview CVS Health is a health solutions company that provides an integrated healthcare services to its members.
