Virtual Nurse Coach – LPN, Anywhere
The Role: Remote Virtual Nurse Health Coach Key Job Functions: • Welcoming patients to benefits of remote monitoring • Educating patients on equipment they will use • Near real time review of patient vital signs • Obtaining actionable information from patients with abnormal readings • Using practice EMR to escalate patient situations to the provider for medical intervention • Reviewing & assisting the provider in the creation of a patient centered care plan • Monitoring notifications and observing trends • Creating patient centered care plans as well as reviewing monthly plans with patients • Monitoring patient compliance with the plan of care & devices • Coaching patients on healthier behaviors • Act as first contact for patient motivationg factors and leveraging them for best possible health outcomes • Proactively managing patients to prevent urgent care or emergency room visits Required Experience: • 2 years nursing experience; case management or home health preferred • Active state licensure (compact licensure preferered) • EMR Experience • Computer competency • Experience caring for some or all of following conditions: • Hypertension • Diabetes • COPD • Asthma • CHF • A Fib Location: Remote, working from home Job Details: • Full-time, salary position • Full benefit package available • Software and hardware provided upon hire About Remote Care Partners: Remote Care Partners (RCP) provides healthcare providers with a cloud-based platform for population management.
