Job Highlights: Start Date: Immediate openings available Compensation: A competitive salary ranging from $54,095.60 to $116,760.80 per year Company: CVS Health Location: Remote (must live in North Carolina) Position: Health Coach Consultant RN Chronic Condition Coach Position Summary We are seeking a highly skilled and compassionate Health Coach Consultant RN to join our team in North Carolina. As a Health Coach Consultant RN, you will work with our members to assess their health needs, develop personalized plans to achieve optimal health, and provide ongoing support and education to help them make informed decisions about their health. This is a full-time, 40-hour per week position with a flexible schedule that includes rotating weekends and evenings. Key Responsibilities: Assess members' health needs through comprehensive evaluation of clinical data and information Develop and implement personalized plans to help members achieve optimal health status Provide ongoing coaching, education, and support to members to promote healthy lifestyle choices and behavior changes Utilize clinical guidelines and criteria to positively impact members' health outcomes Collaborate with supervisors, Medical Directors, and other stakeholders to overcome barriers to meeting goals and objectives Identify and escalate quality of care issues through established channels Document clinical notes and plans of care in an electronic medical records (EMR/EHR) system Requirements Essential Qualifications: Unrestricted RN compact license in the state of North Carolina 3+ years of recent clinical experience in Adult Medical Surgical floor nursing 3+ years of recent clinical experience providing health education about chronic health conditions to adults 3+ years of experience with documenting clinical notes in an electronic medical records (EMR/EHR) system 3+ years of proficiency with Microsoft Office applications: Word, Outlook, Excel, Teams, and PowerPoint 2+ years of experience delivering health education coaching via telephone 1:1 Associate's or Bachelor's degree in Nursing Preferred Qualifications: Previous telephonic chronic condition coaching experience Virtual group coaching experience via webcam Managed care experience Experience with Epic EMR database functionality and documentation Excellent communication skills and comfort with public speaking Ability to work independently as well as part of a group What We Offer Compensation and Benefits: Competitive salary ranging from $54,095.60 to $116,760.80 per year Eligible for a CVS Health bonus, commission, or short-term incentive program Full range of medical, dental, and vision benefits 401(k) retirement savings plan and Employee Stock Purchase Plan Fully-paid term life insurance plan Short-term and long-term disability benefits Paid Time Off (PTO) or vacation pay Paid holidays throughout the calendar year Career Growth Opportunities: Opportunities for professional growth and development Free development courses and education assistance CVS store discount and discount programs with participating partners Work Environment and Culture: Remote work opportunity with flexible schedule Collaborative and supportive team environment Opportunities for work-life balance How to Apply If you are a compassionate and skilled Health Coach Consultant RN looking for a new challenge, we encourage you to apply for this exciting opportunity.
