Job description: JOB SUMMARY: Assists in meeting the psychosocial needs of patients and enables patients and families to utilize medical care to achieve their optimal level of health through established programs within the Case Management Department. The Medical Social Worker also insures effective utilization of the health care system for the benefit of the patient. Performs other related duties as deemed competent.
QUALIFICATIONS: Responsible for providing patient discharge planning needs based on assessment. Meets the post discharge needs of the patient and family in providing collaborative patient services and intervention in discharge planning with post-hospital providers including long term care facilities, Home Health, equipment companies, etc. Completes appropriate transfer forms such as FL-2s, PASARR’s, and discharge planning assessments. Works under the direction of the Director of Case Management. Insures effective utilization of the health care system for the benefit of the patient. Performs other related duties as deemed competent.
ESSENTIAL FUNCTIONS: * Ensure that patients have a safe discharge plan with access and knowledge of supportive resources.
• Ensure effective use of the health care system, utilizing established computer alerts to identify potential candidates for emergency and acute care overutilization.
• Coordination of care for all ED patients, especially vulnerably and high-risk populations, requiring a high degree of problem solving and effective communication.
• Provides Virtual behavioral health services to emotionally disturbed, mentally ill, and chemically dependent individuals.
• Responds to referrals and request for Behavioral Health Service Line services as well as conducting the appropriate clinical screening for potential admission or service use.
QUALIFICATIONS:
Education: Bachelor of Arts (Accredited School of Social Work) required or combination of experience and education.
Experience: 1 year in a health care setting preferred
Other: Thorough knowledge of Social Work theory and method. Medical terminology knowledge. Firm core preparation in social work practice. Extensive ability to work under stress in crises situations. Excellent communication and writing skills required. Knowledge of Medicare/Medicaid guidelines related to discharge planning. Thorough knowledge of governmental regulations in regard to insurance reimbursement with Medicare/Medicaid guidelines. BLS required.
TYPICAL PHYSICAL DEMANDS: Requires functional range of motion. Functional hearing and vision. Occasional sitting, standing, walking. Occasional bending, stooping, and stretching. Lifting/carrying/pushing/pulling 20 pounds.
