In this role, you will play a crucial part in supporting patients and their families through the challenging stages of Chronic Kidney Disease (CKD) and End Stage Kidney Disease (ESKD), including end of life, ensuring they receive the highest quality of care and emotional support.

Key Responsibilities:

Team Social Worker: Work directly with patients and families (70% of the time). Patient and Family Support: Provide behavioral health support to high risk patients and their families, helping them navigate the complexities of kidney disease, and increase coping. Resource Coordination: Connect patients and families with community resources and support services, including financial assistance, behavioral health services, and care needs services. Advance Care Planning: Assist patients and families with goals of care conversations, and provide education on the advance care planning process. Education and Advocacy: Educate patients, families, and caregivers about available palliative and hospice care services, advocating for patient needs and preferences. Team Collaboration: Collaborate with interdisciplinary teams to develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients. Liaise with dialysis clinic staff and outside providers (30% of the time).

Qualifications:

Education: Master's degree in Social Work from an accredited institution.

Licensure: State licensure as a Licensed Clinical Social Worker (LCSW) required.

Experience: At least 2-4 years of clinical experience, preferably in healthcare settings. 2 years of renal experience preferred. Previous behavioral health experience and/or hospice or palliative care experience preferred. Bilingual preferred.

Skills: Strong interpersonal and communication skills, ability to provide empathetic support in a remote work setting.

Knowledge: Familiarity with end-of-life care principles, grief counseling techniques, behavioral health interventions, and community resources.
