About the Senior Community Liaison Role We're seeking an experienced Senior Community Liaison to join our team, focusing on Home Health and Hospice. As a Senior Community Liaison, you will be the face of HarmonyCares in your local market, building relationships with referral sources, patients, and caregivers. You'll play a critical role in promoting our services, explaining the benefits of enrollment, and ensuring that patients receive the care they need. Key Responsibilities Conduct market analysis and understand the competitive landscape to differentiate HarmonyCares' services Meet and exceed face-to-face and virtual meeting goals with key decision-makers Develop a deep understanding of our eligibility and enrollment processes Track performance in CRM, demonstrating a plan and adherence to budgeted targets Schedule and conduct discussions with patients, POAs, or guardians to guide them through the enrollment process Provide direction on benefit enrollment programs and processes Obtain Voluntary Alignment Forms from patients, POAs, or guardians Respond to inbound calls from patients, families, and facility staff regarding our programs Triage patient concerns related to care needs, scheduling, and billing Oversee and document outreach calls and meetings with external sources Ensure timely attendance at scheduled meetings with patients and caregivers Adhere to company policies and procedures, including PHI and HIPAA guidelines Mentor established and new Community Liaisons on sales techniques, reporting, and other tasks Travel up to one week per month to provide onsite assistance to Community Liaisons Comply with STARK Laws as they pertain to healthcare sales Essential Qualifications To succeed in this role, you'll need: A Bachelor's Degree in Marketing, Business, or a Health-related field A valid driver's license and good driving record 3+ years of healthcare-related industry sales/marketing experience Excellent written and verbal communication skills The ability to work independently and manage multiple tasks Strong organizational skills Proficiency in Microsoft Office, including Word, Excel, Outlook, and Report software A proven track record of building strong telephonic relationships with patients Preferred Qualifications We're looking for someone who is: Self-motivated and driven, with a high level of performance even with minimal oversight Experienced in sales and marketing within the healthcare industry What We Offer At HarmonyCares, we believe in rewarding our team members for their hard work and dedication.
