Job description: Building Location: Grace Home Nursing HomeDepartment: 13091 Nursing Home-Historic (inactive)Job Description: Seeking a Registered Nurse or Licensed Practical Nurse to serve at Grace Home, Essentia Health's long-term care facility in Graceville, Minnesota. This RN or LPN works 6:00 am to 6:30 pm, 36 hours weekly. Every third weekend and a holiday rotation are required.Education Qualifications:

Contributes to the meeting of the mission and goals of the facility and Essentia, and meets the requirements of the Joint Commission if applicable and/or other federal, state, and local regulatory or accrediting agencies.LPN ($24.94-$37.41 based on experience):A care giver who assumes responsibility and accountability for a group of primarily elderly patients for a designated time frame and provides care to these patients via therapeutic use of self, the nursing process, the environment/instrumentation, and other health care team members.
