Job Title: Remote RN Case Manager-Bilingual in Spanish
Duration: 6 Months (Extension Possible)
Pay Rate: $48/hr (W2)
Location: 100% Remote (Open to all U.S. time zones; CST preferred for 1–2 candidates)
Work Authorization: Must be a U.S. Citizen

Job Overview:

We are seeking a skilled Remote RN Case Manager to join our team on a 6-month contract, with potential for extension. This role is focused on ensuring that members with complex medical or psychosocial needs receive high-quality, cost-effective care. The RN Case Manager will perform holistic assessments and collaborate with internal and external stakeholders to develop and implement individualized care plans.

Key Responsibilities:
• Conduct telephonic interviews using motivational interviewing techniques
• Collaborate with case management team, physicians, and interdepartmental staff
• Educate members using approved tools to support care plan adherence
• Collect and evaluate medical data to identify unmet needs
• Create and monitor individualized care plans
• Coordinate with providers, home health, and community resources
• Participate in interdisciplinary team meetings and quality initiatives
• Maintain accurate documentation and adhere to regulatory guidelines
• Remain informed about benefit design, cost analysis, and community resources
• Promote collaboration with external payers, agencies, and partners
• Participate in internal training and continuing education

Required Qualifications:
• RN License with Bachelor’s degree in Nursing (or equivalent RN certification)
• Minimum 3 years of experience in Acute or Managed Care (Medicaid/Medicare populations preferred)
• 3–5 years of experience in Acute Care/Managed Care settings
• Telephonic case management experience

Preferred Qualifications:
• Bilingual in English/Spanish
• Case Management Certification (CCM or similar)

• 100% remote role
• Must be willing to complete the Care Manager Assessment via Glider
• Must be a U.S. Citizen
