RN Case Manager - Full-Time - Mandan, ND
Job Summary:
Join our team as a full-time RN Case Manager at Sanford Health in Mandan, ND. As a vital member of our healthcare team, you will integrate healthcare services for clients from pre-admission to post-discharge, ensuring seamless coordination and communication among patients, families, and healthcare professionals.
Key Responsibilities:
Coordinate and sequence client care to enhance patient flow and interdisciplinary communication
Evaluate outcomes based on clinical, financial, and utilization data
Utilize independent nursing judgment in structured and unstructured healthcare settings
Communicate effectively with patients, families, and healthcare teams
Monitor, evaluate, and trend patient responses using structured care methodologies
Fiscally responsible and able to control variances and duplication
Demonstrate independence, assertiveness, and critical thinking in patient care and team collaboration
Requirements:
Bachelor's degree in nursing preferred
Graduate from a nationally accredited nursing program (CCNE, ACEN, or NLN CNEA)
1-2 years of professional nursing experience in a clinic care setting preferred
Unencumbered RN license in the State Board of Nursing where practice occurs
Multistate licensure if in a Nurse Licensure Compact (NLC) state
Valid driver's license may be required depending on location
