RN - assessment & service coordination consultant (hybrid)
About the position Responsibilities • In-person and virtual training and technical assistance with Service Coordinators • Applying subject matter expertise with the interRAI-HC tool • Ensuring best practices are used in the administration of the interRAI-HC tool • Conducting quality reviews of assessments and service plans • Proficiency testing • Developing and implementing performance improvement plans and monitoring efficacy • Collaborating with supervisors, managers, trainers, and educators • Data analysis and reporting Requirements • Registered Nurse with experience in conducting functional assessments of older adults and adults living with disabilities • Experience in Home and Community Based Services (HCBS) or Long Term Services and Supports (LTSS) • Experience in case management and service coordination • Experience in leadership, management, supervision, or consulting • Experience in providing technical assistance and training • Subject matter expertise with the interRAI-HC tool is strongly preferred Benefits • Annual salary of $90,000 • Health insurance • Dental insurance • Vision insurance • Life insurance • Disability insurance • 30 days of paid time off each year for holidays, vacation, sick leave, and personal days • Health Savings Account • 401k plan • Other perks Apply tot his job
