As a remote Triage LVN, you will play a critical role in our team, ensuring that our patients receive the support and guidance they need, even on weekends and evenings. Key Responsibilities: Providing Expert Remote Triage Support Respond to Incoming Calls: Answer all incoming phone calls during your scheduled on-call hours, providing timely and compassionate responses to patients, caregivers, and healthcare facilities. Coordinate Patient Care: Work closely with our nursing staff to dispatch field nurses for patient visits as necessary, ensuring that our patients receive the care they need in a timely manner. Emotional Support and Guidance: Offer emotional support and practical care guidance to families and caregivers, helping them navigate the challenges of end-of-life care. Apply Triage Protocols: Utilize established triage protocols to assess the urgency of patient needs and determine the most appropriate course of action. Documentation and Communication: Document all triage activities, calls, and interventions using our company-provided software and communication tools, ensuring seamless communication with the interdisciplinary team. Collaboration with the Interdisciplinary Team: Communicate effectively with our team regarding patient needs and updates, ensuring that our patients receive comprehensive, coordinated care. Requirements: The Ideal Candidate To excel in this role, you will need: Active LVN License: A current LVN license in the state of California, demonstrating your expertise and commitment to the nursing profession. Hospice Experience: A minimum of 1 year of experience in hospice care or a similar healthcare setting, equipping you with the knowledge and compassion required to support our patients and their families. Strong Communication and Organizational Skills: Excellent communication and organizational abilities, enabling you to manage multiple priorities, communicate effectively with our team, and provide clear guidance to patients and caregivers. Independence and Decision-Making: The ability to work independently and make informed decisions during triage situations, ensuring that our patients receive timely and appropriate care. Preferred Qualifications While not required, the following qualifications can enhance your application: Experience with Hospice Software: Familiarity with hospice care software and communication tools, facilitating efficient documentation and communication. Previous Remote or On-Call Experience: Experience working in remote or on-call positions, demonstrating your ability to work flexibly and respond to patient needs outside traditional office hours.
