Remote Senior Care Coordinator - Patient Support

Key Responsibilities:
Conduct comprehensive needs assessments for seniors via video calls. Develop, implement, and manage personalized care plans for clients. Coordinate in-home care services, including scheduling caregivers and monitoring their performance. Liaise with healthcare providers, specialists, and other community resources to facilitate comprehensive care. Serve as the primary point of contact for clients and their families, addressing inquiries and concerns promptly. Monitor client progress and well-being, making adjustments to care plans as needed. Maintain accurate and confidential client records in the designated system. Ensure compliance with company policies, procedures, and relevant regulations. Educate clients and families about available services and support options. Advocate for clients' needs and promote their independence and quality of life.
Qualifications:
Bachelor's degree in Social Work, Nursing, Gerontology, Healthcare Administration, or a related field. Minimum of 3-5 years of experience in case management, social work, or healthcare coordination, preferably with a focus on geriatrics or senior care. Strong understanding of senior care needs, chronic conditions, and community resources. Excellent communication, interpersonal, and active listening skills. Proficiency with client management software, EHR systems, or similar platforms. Strong organizational and time management abilities, with the capacity to manage multiple caseloads.
