🥠Remote RN for Virtual Patient Care (Bilingual EN/SP Preferred)

Part-Time Flexible Hours | US-Based Only Lara Health is seeking a compassionate and experienced Registered Nurse (RN) to join our Chronic Care Management (CCM) / Remote Patient Monitoring (RPM) team. This is a fully remote role with flexible scheduling, ideal for someone passionate about chronic care and patient engagement. 💼 Position Highlights • Schedule: Minimum 20 hours/week • Location: Fully remote (U.S. based only) • Experience Required: Minimum 5 years caring for patients with chronic conditions (e.g., diabetes and HTN, cardiology, primary care) П©º Role Responsibilities • Lead and coordinate virtual care efforts among Medical Assistants, LPNs, and other team members • Serve as a clinical point of contact for escalations and care plan adjustments • Collaborate with providers to ensure continuity and quality of care across patient touchpoints • Monitor team workflows and contribute to improving efficiency and patient outcomes • Participate in virtual team huddles, case reviews, and care strategy sessions • Support onboarding and mentorship of new care team members • Monitor patient vitals (blood pressure, blood sugar, etc.) and medication adherence • Support patients in scheduling MD visits and making lifestyle changes • Maintain accurate and up-to-date EMRs • Collaborate with providers and care team • Perform other duties as assigned 🠕 Required Skills & Qualifications • Licensure: Active RN license (mandatory) • Experience: 5+ years in nursing, preferably in chronic care settings • Language: Bilingual English/Spanish strongly preferred • Strong communication, organizational, and time management skills • Ability to work independently and collaboratively • Proficiency with computers and EMR systems (must have own device and reliable internet) • Familiarity with medical terminology and clerical tasks • Passion for working with older adults and delivering preventative care П“‹ Preferred Experience • Prior experience in care team leadership or practice management • Familiarity with remote care coordination tools and EMR-based task management • Ability to delegate, prioritize, and oversee multiple patient cases simultaneously 🚫 Please Note • Do not apply if you do not meet the required qualifications • No agencies or recruiters • Online applications only — please do not call or email • Only shortlisted candidates will be contacted Apply tot his job
