This role offers an unparalleled opportunity to lead a new telehealth-based clinical team focused on managing complex Medicaid populations in Arizona. You will work remotely with a collaborative, interdisciplinary team to improve patient outcomes and reduce healthcare costs through innovative, coordinated care.
This position is ideal for an NP with strong leadership skills who thrives in a dynamic startup environment and is deeply committed to caring for vulnerable populations. We are hiring for both a Nurse Practitioner and a Lead Nurse Practitioner position (with higher compensation for the Lead role).
Requirements
• 1+ years of experience as a licensed Nurse Practitioner
• Active/clear Nurse Practitioner license to practice in the State of Arizona
• Active/Clear DEA
• 1+ years of experience working in a rehab setting /rehabilitation facility centers
• Strong knowledge in HIPAA compliance as it pertains to telehealth
Key Responsibilities
• Serve as the operational lead for the interdisciplinary care team.
• Lead care coordination efforts for complex Medicaid populations.
• Practice autonomously with prescribing authority.
• Manage care delivery primarily through telehealth and remote coordination.
• Coordinate care across multiple providers, including primary care physicians and health plan case managers.
• Support proactive care management, including risk stratification and early detection of health declines.
• Provide 24/7 clinical access to help patients avoid unnecessary Emergency Room visits.
• Support patients during hospitalizations and post-discharge transitions.
• Help build and shape a new, innovative value-based care model.
Schedule and Compensation
• Schedule: Primarily a virtual care model with collaborative interdisciplinary teams.
• Compensation: The base salary can range from $130,000 to $175,000, depending on experience and whether the role is a Leader position.
