Remote Medical Assistant – Chronic Care Management (CCM)

About the Role: We are looking for a compassionate and organized Medical Assistant to join our Chronic Care Management (CCM) team. In this role, you will connect with patients monthly to review their health goals, medication adherence, and care plans — helping them stay on track between doctor visits. This is a fully remote position ideal for someone who enjoys building relationships with patients and making a real impact on their quality of life. Responsibilities: • Call and engage patients each month to provide ongoing chronic care management services • Review and update patient care plans, medications, and health goals • Document all interactions accurately in the electronic health record (EHR) system • Communicate care updates or concerns to the supervising provider • Ensure patient satisfaction and adherence to their care plan • Participate in regular team meetings and compliance training Qualifications: • Certified or Registered Medical Assistant (MA, CMA, RMA) • Minimum 1 year of experience in a medical office, telehealth, or care coordination setting • Excellent phone etiquette and communication skills • Strong documentation skills and attention to detail • Reliable internet connection, computer, and quiet workspace • Experience with EHR systems preferred (training provided) Benefits: • 100% Remote – work from home anywhere in the U.S. • Flexible scheduling • Full training provided • Supportive, patient-focused team environment Job Types: Full-time, Contract Pay: $15.00 per hour Expected hours: 40 per week Benefits: • Professional development assistance Work Location: Remote Apply tot his job
