Remote LVN (Outreach Team)
An employer is hiring for a remote LVN that currently holds a multi-state compact license and has supported Chronic Care Management. Your primary focus will be helping patients through the care consent process : connecting with them, explaining our client's services, and guiding them into active care. You'll begin with outreach-specific responsibilities, with the opportunity to grow into providing direct patient care after successful performance.
ā¢ Contact patients and caregivers to explain the client's care services and walk them through the consent process
ā¢ Clearly communicate what our care program offers and how it benefits patients with neurological conditions
ā¢ Follow structured outreach workflows with precision and attention to detail
ā¢ Document interactions thoroughly and accurately
ā¢ Maintain a patient-friendly, warm, and professional tone in every interaction
ā¢ Collaborate with the care team and engineering / growth teams to improve outreach processes
ā¢ Provide feedback on outreach tools and workflows to help us iterate quickly
ā¢ Day shifts : Depending on local time zone and clinics you're supporting
ā¢ 7 : 00 AM - 3 : 00 PM
ā¢ 8 : 00 AM - 4 : 00 PM
ā¢ 9 : 00 AM - 5 : 00 PM
ā¢ Overtime opportunities available
ā¢ Evening shift (2 positions) :
ā¢ 5 : 00 PM - 9 : 00 PM

Required Skills & Experience
ā¢ Active LVN / LPN license (multi-state compact)
ā¢ Organized and able to learn quickly on the job while maintaining systems
ā¢ Knowledge of CCM (chronic care management)
ā¢ Must have a current desktop or laptop at home that you can use to start while we work to get a company laptop
Nice to Have Skills & Experience
ā¢ Has worked remote before
