Remote Licensed Practical Nurse (LPN) - Full-time/Part-time Opportunities

As a Licensed Practical Nurse, you will play a vital role in our Medication Management and Care Coordination department, conducting telephonic patient outreach and interviews to ensure adherence and patient safety.
About the Role:
Perform telephonic patient interviews in English and Spanish to discuss medications, adherence, and health status
Provide therapeutic recommendations and accurately document clinical data in patient charts
Identify potential issues and maintain current knowledge of medications, HIPAA, and CSS Health policies and procedures
Collaborate with our team of pharmacists, clinicians, and support staff to deliver exceptional patient care

Requirements:
Graduation from an accredited nursing program (LPN or LVN) with an active license in good standing
