Ā Advocate for members to ensure their preferences and requirements are prioritized by the healthcare team.
Ā Maintain clear and continuous communication with all relevant stakeholders to support effective care coordination.

Qualifications
Ā Current and unrestricted Massachusetts RN license.
Ā Minimum of 2 years of clinical experience.
Ā Proficiency in Microsoft Office Suite (Word, Excel, Outlook).
Ā Willingness to travel within the designated region to conduct visits with members.
