Job Description:
The successful candidate will be responsible for:
Leading and supervising a team of care management staff, providing guidance, support, and performance feedback
Overseeing caseloads, ensuring timely communication with members, providers, and community-based organizations
Providing training, resolving staff issues, and identifying areas for program improvement
Collaborating with management to perform annual reviews, disciplinary actions, and goal setting
Establishing and sustaining relationships with community systems to facilitate linkage, referral, advocacy, and follow-up
Assisting with member concerns and grievances, and participating in internal and external audits
Skills and Qualifications:
To be successful in this role, you will need:
Excellent communication, negotiation, and interpersonal skills
Ability to work effectively in a cross-functional team environment
Strong computer literacy and skills
Self-motivation to seek knowledge and improve personal and professional performance
Regular attendance and a strong work ethic
Preferred qualifications:
Master's degree
Spanish language proficiency (strongly preferred)
