Registered Nurse (RN) – Remote Patient Monitoring (Heart Failure/Cardiology) Position Overview We are seeking a compassionate and detail-oriented Registered Nurse (RN) to support patients enrolled in a Remote Patient Monitoring (RPM) program. This role focuses on virtual engagement, proactive data review, and patient education for individuals with heart failure and related cardiac conditions. The RN will collaborate closely with a multidisciplinary clinical team to ensure timely interventions, continuity of care, and high-quality patient outcomes. Key Responsibilities • Utilize the RPM platform to monitor patient data, identify trends, respond to alerts, and engage patients with personalized, real-time interventions. • Collaborate with the clinical care team to coordinate and deliver virtual care while maintaining high clinical standards. • Provide patient education on heart failure management, medication adherence, and lifestyle modifications to support care plan compliance. • Address clinical alerts and patient messages promptly, offering guidance in accordance with established protocols. • Regularly assess RPM data and patient-reported outcomes, escalating concerning findings to the care team as appropriate. • Maintain accurate and timely documentation of patient interactions, clinical notes, and follow-ups in compliance with regulatory, provider, and platform requirements. Required Qualifications & Skills • Active Registered Nurse (RN) license in the state of Massachusetts (required) • Compact RN License (preferred) • Prior experience in Heart Failure and/or Cardiology • Ability to deliver patient care and education effectively in a virtual/remote setting • Strong written and verbal communication skills • Ability to work independently in a quiet, private home workspace • Comfort using digital platforms, remote monitoring tools, and electronic documentation systems Work Environment • Fully remote / virtual care setting • Must have a reliable internet connection and a distraction-free workspace Apply tot his job
