Job Responsibilities:
Conduct telephonic clinical assessments using approved medical protocols
Provide recommendations for appropriate levels of clinical care based on clinical judgment and protocols
Outreach and engage members with programs to promote health and well-being
Complete clinical eligibility assessments and identify health, condition, or social gaps
Work Schedule:
Monday-Friday: 2 shifts Pacific Time Zone between 8:30 am - 5 pm, then 3 shifts Pacific Time 10:30 am - 7 pm. Quarterly Saturday shifts 8:30 am - 5 pm Pacific time.
Requirements:
Undergraduate degree or equivalent experience
Multi-State Compact License
Outpatient clinical, ICU, ER, PCU, Step-down experience
Computer proficiency (Microsoft Word, Outlook, and internet)
Preferred Qualifications:
Call center experience
Experience with EMR (Electronic Medical Records)
Recent Case Management experience
Bilingual - Spanish
