ā¢ Iowa or Oklahoma: Fully Remote
Overview:
Seeking Registered Nurses (RNs) to join a new, high-impact program supporting individuals with Intellectual and Developmental Disabilities (IDD) enrolled in Colorado Medicaid. In this role, you will conduct face-to-face and/or virtual clinical assessments of members to evaluate their medical, personal, and caregiver support needs. Key Responsibilities:
ā¢ Conduct 2-3 hour assessments with IDD members and caregivers
ā¢ Use skilled care acuity tools to assess needs related to:
ā¢ Clinical/medical status
ā¢ Personal care and daily living support
ā¢ Caregiver assistance
ā¢ Document findings thoroughly and accurately in internal systems
ā¢ Collaborate with healthcare providers to ensure coordinated care
ā¢ Mentor new assessors, conduct quality reviews, and participate in process improvements
ā¢ Provide education and resources to members and their families
Minimum Qualifications:
ā¢ MUST RESIDE IN Colorado, Iowa or Oklahoma
ā¢ Bachelor's Degree in Nursing (BSN) required
ā¢ Active RN license in Colorado or compact state
ā¢ 3+ years of nursing experience within the last 10 years (excluding clinicals)
ā¢ Experience working with individuals with IDD
ā¢ Strong understanding of clinical assessments and medical terminology
ā¢ Comfortable conducting assessments via video and in-home visits
Preferred Qualifications:
ā¢ Experience with pediatric and geriatric IDD populations
ā¢ Bilingual (Spanish) strongly preferred
ā¢ Strong critical thinking and attention to detail
ā¢ Ability to work independently while maintaining empathy and professionalism
ā¢ Flexibility in schedule to accommodate patient needs
