Registered Nurse - Virtual patient care

WellSpan Health is seeking a dedicated and compassionate Registered Nurse (RN) – Virtual Patient Care to join our innovative remote healthcare team. This role allows you to leverage your clinical expertise in a virtual setting, providing high-quality care, education, and support to patients from the comfort of your home while ensuring exceptional outcomes.

Responsibilities:

As a Virtual RN, you will:

• Conduct remote patient assessments via phone or video.


• Monitor and manage patients with chronic conditions and post-hospitalization needs.


• Educate patients and families on care plans, medications, and self-management.


• Collaborate with physicians, specialists, and care teams for seamless care coordination.


• Utilize telehealth technology to document and track patient progress.


• Triage patient symptoms and provide guidance based on clinical protocols.


• Address patient concerns, escalating issues when necessary.


• Ensure compliance with HIPAA, patient safety, and regulatory guidelines.


• Participate in virtual team meetings and continuous professional development.

Requirements:

To be successful in this role, you should have:

• A valid RN license in [State(s) required].


• Minimum of two years of clinical experience, preferably in telehealth, primary care, or case management.


• Strong communication and critical-thinking skills.


• Ability to work independently and efficiently in a remote setting.


• Proficiency in electronic health records (EHR) and telehealth platforms.


• Experience with triage protocols and virtual patient assessments.


• Reliable internet connection and a dedicated workspace.


• A passion for patient advocacy and education.


• Flexibility to work shifts, including evenings or weekends if needed.
