Responsibilities: As a Hospice Care Case Manager at Amedisys, your primary responsibilities will include: Patient Assessments: Perform comprehensive patient assessments to identify their unique needs and develop a plan of care. Care Planning: Collaborate with the care team to develop and implement a plan of care that addresses the patient's physical, emotional, and spiritual needs. Patient Education: Teach patients and their families about self-care techniques, disease management, and end-of-life care. Care Coordination: Supervise LPNs and HHAs, and coordinate care with other healthcare professionals. Documentation: Complete accurate and timely documentation of patient care and progress. Communication: Regularly communicate patient progress to the clinical manager and care team. On-Call Responsibilities: Participate in on-call rotations and provide care to patients and families as needed. Qualifications: To be considered for this rewarding role, you'll need: Current RN License: A current RN license in the state(s) you are assigned to work. Clinical Experience: At least one (1+) year of clinical experience as a Registered Nurse. If less than 1 year of clinical experience, candidate must be approved by VP Clinical. CPR Certification: Current CPR certification. Valid Driver's License: A valid driver's license, reliable transportation, and liability insurance. RN Intern Program: If less than 6 months of clinical experience as a RN, candidate must participate in RN Intern program.
