PRN Nurse

Deliver an exceptional end-of-life experience, one patient at a time, and raise the standard of hospice care everywhere we serve. The PRN Nurse is a growth role to serve patients on our census. This position could work during the day throughout the week or cover on-call nights and weekends occasionally.
Responsibilities
• Provide comprehensive care coordination and support to hospice patients and their families.
• Assess patient needs, develop individualized care plans, coordinate care services, and provide emotional support throughout the end-of-life journey.
• Clinical Assessment and Care Planning
• Symptom Management and Pain Control
• Emotional Support and Counseling
• Care Coordination and Collaboration
• Documentation and Compliance
Requirements
• RN licensure for the state of Indiana
• Clean driving record, and valid driver’s license and auto insurance
