The role involves delivering high-quality, documented care in a compassionate and empathetic manner, ensuring all activities are in compliance with physician orders and established care plans. This is a night shift position.Key Responsibilities
Provide and coordinate quality patient care in compliance with a physician's orders.
Observe and assess patient conditions and needs continuously, reporting any changes to the supervisor or physician.
Document all activities, assessments, and nursing actions in a timely and confidential manner.
Participate in the implementation and updating of the patientβs nursing care plan.
Take appropriate nursing actions to achieve expected patient outcomes and prevent or reduce risk.
Educate patients and their families about disease processes, self-care techniques, and prevention strategies.
Comply with all policies, state and federal regulations, and professional standards.
Maintain a commitment to infection control measures and professional development.
Requirements
A current, unrestricted license as a Licensed Practical or Vocational Nurse in the state of practice.
A graduate of an accredited school of nursing.
Current CPR certification with a hands-on component.
Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing policies.
A recent TB skin test (within the last 12 months).
Reliable transportation.
Compliance with all health-related requirements, including required vaccinations and testing.
Physical ability to lift up to 50 pounds, reposition patients, and move equipment.
Preferred Qualifications
Six months of recent experience as a Licensed Nurse in a clinical care setting is preferred, but not required.
Experience in home health care.
Depending on the state of practice, one year of experience may be required (e.g., South Carolina, California, Louisiana).
