Position Overview:

We are hiring a Full-Time Nurse Practitioner to serve as a cover provider within our post-acute care team. This role requires someone flexible, dependable, and thrives in autonomous clinical environments. You will:
• Cover SNF patients when regular providers are out (vacation, sick leave)
• See long-term care residents as needed
• Carry a panel of low-acuity or non-skilled patients at a designated LTC facility
• Represent Paragon Health Partners professionally and embody our core values at every facility
• Engage in accountable, results-driven teamwork with open feedback and communication

Key Responsibilities:
• Deliver compassionate and compliant care in SNFs and LTC facilities
• Attend and document weekly therapy meetings in patient records
• Join weekly Post-Acute Care Division L10 meetings
• Document and review issues and team updates weekly
• Complete all documentation within:
• 24 hours for daily rounds
• 72 hours for consultation/admissions
• Follow all clinical protocols related to pain management, post-acute care, and buprenorphine treatment

Qualifications:
• Active Nurse Practitioner license in Texas
• Experience in SNF, LTC, or post-acute care preferred
• Ability to work autonomously while remaining a strong team player
• Effective communicator with strong documentation habits
• Experience with pain management or physiatry is a plus

Pay: $130,000.00 - $180,000.00 per year
