• Physical Therapist – In-Home Health Mobility & Functional Rehab Specialist*
• Location:* Concord, CA
• Salary:* $148,000–$160,000 per year
• Schedule:* Full-Time | Field-Based | Flexible Hours
• Overview*
• P3SG* is seeking a motivated *Home Health Physical Therapist (PT) *for its client to support patients across the Bay Area. This hybrid role blends *in-home patient visits*, remote flexibility, and a mixed *hourly + points compensation model*. Ideal for therapists who value autonomy, mobility-focused care, and meaningful patient relationships.
• Key Responsibilities*
• Perform *home health physical therapy evaluations* and functional mobility assessments.
• Develop individualised *rehabilitation and treatment plans* to promote independence and safe movement.
• Deliver person-centred therapeutic interventions in the home setting.
• Participate in *interdisciplinary team meetings* and care coordination.
• Complete accurate, timely *clinical documentation* following home health and CMS guidelines.
• Identify additional services (OT, ST, MSW) and communicate clinical needs to the care team.
• Provide professional oversight to *Physical Therapist Assistants (PTAs)* when required.
• Qualifications*
• Active *California Physical Therapist License*
• Graduate of an accredited PT program
• Previous PT experience (home health, outpatient, or rehab experience welcomed)
• Current CPR certification
• Reliable, insured vehicle + valid driver’s license
• Strong charting/documentation skills (EMR experience helpful)
• Ability to work independently with changing daily routes and patient needs
