As a Patient Access & Referral Specialist, you will:
Conduct mobile assessments and review for clinical disposition.
Provide crisis intervention support.
Triage patients as needed.
Complete assessments and provide referrals as appropriate.
Collaborate closely with medical staff, treatment teams, and managed care organizations.

Requirements:
Master's Degree in Social Work or related field; or Registered Nurse license in the State of Indiana.
Proficient in computer applications.
Excellent listening and communication skills.
Strong organizational and time management abilities.
Ability to work independently and as part of a team.
