PART - TIME, AS NEEDED POSITION
MedKick is a growing and dynamic medical company with multiple locations throughout the country. This is a great opportunity for medical professionals with excellent communication skills, who have knowledge of chronic conditions such as diabetes, cardiovascular disease, and COPD, and have a desire to make positive differences in many patients lives. Develop relationships with patients, no longer just passing a medication and feeling pressure of being on the floor.
General Summary:
Seeking a full-time Chronic Care Coordinator to provide Chronic Care Management (CCM) and/or Remote Patient Monitoring (RPM) to patients, with multiple comorbidities, or those with certain chronic conditions, including but not limited to, diabetes, cardiovascular disease, and COPD. This is a remote position and involves being on the phone with patients the majority of your day. You work with the same patients from one office month after month.
This role is responsible for assessing and interpreting clinical information and accurately documenting patient assessments to develop care plans for patients participating in the Programs. The Coordinator primary focus is improving patient health, experience, ensuring high-quality outcomes for patients with chronic diseases and reducing unnecessary utilization of hospitalization or ER visits.
Minimum Qualifications:
· Experience working with geriatric patient populations, with training in chronic disease management with a focus on conditions such as HF, Diabetes, COPD, etc. Prior experience working with Chronic Care Management program highly desired, please indicate that in a private message if possible.
· Certified or Registered Medical Assistant, LPN or RN licensed is required.
· Must possess excellent interpersonal, communication and customer service skills.
· Passionate about patient care, possessing the ability to relate with empathy and compassion. A passion for patient teaching is a must.
· Must be able to work in a seated position for extended periods of time as this is primarily a telephonic and computer function.
· Proficient computer skills, including in Electronic Health Records and Microsoft programs.
· Work hours are usually 8:30 a.m. 5 p.m, semi-flexible
