Give your passion a purpose – join our amazing team! At Lower Cape Fear LifeCare, your passion finds purpose every day. As an award-winning nonprofit hospice and palliative care provider, we've been serving families across southeastern North Carolina and northeastern South Carolina for over 40 years—rooted in heart, service, and community. What sets us apart? Our commitment to your well-being and growth: Quick Benefits & Affordable Coverage: Health, dental, and vision insurance begin the first of the month after 30 days. We offer a free employee-only health plan option, and all plans include access to virtual doctor visits and mental health support at no cost. Generous Paid Time Off: Full-time employees start with 27 days of PTO in their first year—plus it rolls over and increases with tenure. Investing in Your Future: We offer a 403(b) retirement plan with employer match, along with up to $5,000 per year in tuition reimbursement to support your continued growth. A Culture That Values You: We believe in celebrating contributions, uplifting one another, and fostering a workplace built on respect, trust, and true compassion—for our patients, families, and each other. We're proud to rank in the top 94% of healthcare providers nationwide in the Press Ganey employee satisfaction survey. Come see what makes Lower Cape Fear LifeCare a truly special place to grow. Schedule: After your orientation period, your schedule will be seeing patients at their home Monday to Friday, 8:00 AM to 5:00 PM, with dedicated charting time. There is no call or weekend requirements. Since we are not an emergency service, for urgent or emergent situations, patients should be directed to their Primary Care Provider. Clinic Logistics: On an average day, you will see 5 patients – 2 new patients, and 3 follow up patients. Some of these patients may be telehealth visits. New patient visits typically last 1 hour, and follow-up visits are based on provider discretion, usually 30 minutes to 1 hour. What you’ll be doing: You’ll see your patients in a home environment. We provide quality care regardless of circumstance to a variety of different socioeconomic and cultural backgrounds. You’ll need to be comfortable educating the caregivers on how to care for their loved one. This might include things like disease education, advance care planning, how to administer medications, and what to expect at end-of-life. You’ll be provided with company equipment as needed to perform your duties. You will coordinate care between your patient’s community medical providers, and other disciplines to provide updates, obtain new orders. This coordination takes place during and in between visits. You’ll be part of an interdisciplinary team including other providers, nurses, and a medical social worker. We see patients to help with symptom management for serious chronic illness, looking beyond just their diagnosis to help them live life to the fullest. What we're looking for: Education: Graduate of an accredited school with a Master’s Degree in Nursing. Certification in hospice/palliative care preferred. Licensure / Certifications: Current certification from the American Academy of Nurse Practitioners (ANNP) or American Nurses Credentialing Center (ANCC). Must have current license or ability to obtain licensure from the NC/SC Board of Nursing (NP). Experience: Minimum of five years general medical experience in inpatient, outpatient and nursing home settings. Minimum two year experience as a nurse practitioner required. Demonstrated competence in leadership and teaching.
