Job Summary
We are seeking a dedicated and skilled Telehealth Nurse Practitioner to join our healthcare team. In this role, you will provide comprehensive patient care, including assessment, diagnosis of Dementia. You will work collaboratively with physicians and other healthcare professionals to ensure high-quality patient outcomes. The ideal candidate will possess a strong clinical background and demonstrate exceptional communication skills. This is a telehealth position and is fully remote. We require licensure in the following states: Kansas
Duties
• Conduct thorough patient assessments and evaluations to determine health status.
• Develop and implement individualized care plans based on patient needs.
• Perform diagnostic tests and interpret results to guide treatment decisions.
• Provide education and counseling to patients and their families regarding health management.
• Collaborate with interdisciplinary teams in diagnosing and managing Dementia cases.
• Maintain accurate and up-to-date patient records using electronic health record systems such as Athenahealth.
Skills
• Strong background in Geriatric medicine and/ or hospital medicine.
• Experience in psychiatry is desirable.
• Experience working with adults 65+ in a clinical setting is highly desirable..
• Ability to navigate healthcare systems effectively for optimal patient outcomes.
• Excellent interpersonal skills for building rapport with patients and families.
• Strong organizational skills with attention to detail in clinical documentation.
• Kansas NP license is required
