What To Expect (Job Responsibilities)
Engage with patients to navigate their care and address healthcare issues
Collaborate with medical practice staff and the Case Management team
Document patients' medical history and issues in the electronic health record (EHR)
Implement and meet assigned Key Performance Indicators to improve patient outcomes
Utilize various communication methods to engage high-risk patients
What Is Required (Qualifications)
Certified or Registered Medical Assistant OR certified EMT
Experience with medical terminology and understanding of primary care physician practices
Proficiency in using electronic health records (EHRs) and care management analytics
Exceptional written, visual, and verbal communication skills
Ability to work independently as part of a patient's care team under supervision
How To Stand Out (Preferred Qualifications)
Familiarity with electronic health records such as EPIC, Athena, or Cerner
Experience in motivational interviewing and behavioral change management
Strong critical listening skills and empathy in patient coordination
Ability to manage multiple tasks while remaining detail-oriented
Proven success in a remote work environment
